    Dear Pharmacists, 

    Please see below request from our colleagues:

    We are seeking pharmacists who practice in hepatitis C virus (HCV) management to participate in this survey. The purpose of this survey is to further characterize the national HCV workforce by documenting the efforts of pharmacists who manage patients with HCV. 

    If there are multiple pharmacists managing HCV within your institution, please submit their email addresses at the end of the survey so we can send a survey link to them.  

    This survey should only take ~ 5 to 10 minutes to complete. 

    This survey has been sent to multiple pharmacy listservs.  If you have already completed this survey, we thank you for your response, and we kindly ask that you do not complete it again.

    This study has received IRB approval from the University of Illinois at Chicago (The UIC Exempt Research Protocol Number is 2022-0261) and from the University of New Mexico Health Sciences Center (HSC Exempt Research Protocol Number 22-112). Participation in this research survey is completely voluntary and confidential, as data will only be reported in aggregate. Confidentiality will be maintained to the degree permitted by the technology used.  Submission of the responses to the survey will be interpreted as consent to participate.

    We hope that the results of this survey may be used to demonstrate and promote the role of pharmacists in our nation's work toward HCV elimination. We greatly appreciate your support and thank you for your role in HCV elimination.  We thank you in advance for your completion of this survey!
