    Hi,

    I have a lovely patient who will be moving from the SF Bay Area to Cincinatti (Finneytown) within the next several months. I'd love to provide him with contact info for any well-recommended HIV clinics in the area to ensure a smooth transition of care.

    Does anyone have any recommendations? Thanks.

    Greetings - Along the same theme as the previous note - I have a lovely patient relocating to Hagertown MD who will need a new provider.  If anyone knows of a provider there or near to there - I would be grateful to get any referrals for my patient.   Many thanks!

    Hi Carissa and Sara!

    Have you used the Academy's Referral Link to search for providers in those areas?  That can be a way to help get your patients connected with new clinicians.

    My general recommendation is to search by zip code.  The tool then pulls up providers within 100 miles of that code.

    When I checked for Cincinnati, I got a few hits.  For Hagerstown, MD, it looks like the nearest provider in our database is about 45 miles away in Rockville, MD.

    Dan - thanks for the recommendation - I am using the referral finder as well - sometimes though someone might have a personal recommendation and that is always desired.  But thanks for the reminder.
