  • 1.  HIV 1 and 2 testing question

    Please excuse my ignorance, I am just learning, while trying to run a FQHC at the same time. I had a HIV 4th generation screening come back +. It reflexed to antibodies, both negative. It then reflexed to HIV-1 RNA qualitative. I have spoke to the HD and they said false positive. In cases like this, do you do a HIV-2 p24 test? It seems it ruled out HIV-1, but no HIV-2. 
    This may seem like a dumb question, but I don't want to miss anything. 
    Pt is asymptomatic but is a past IV drug user. I want to make sure I am covering all the needed confirmatory tests.

    It is a false positive as you stated. And the HIV 1/2 4th generation tests for both HIV 1 and 2. So you don't need to do a separate test for HIV 2.

    So if the 4th generation tests for HIV1/2 antibodies, and the HIV-1 p24 antigen, there's no testing for HIV-2 antigen? This is where I'm getting a bit confused. It tests for only HIV-1 antigen, what about testing for HIV-2 antigen? I know this is probably a dumb question but I just have to understand the why of it.

    If your HIV RNA is negative, your 4gen rapid was a false positive. No further testing needed now. But if they have ongoing IV drug use I’d get them on PrEP and test every 3 months.
