    ***HEALTH CARE PROVIDER SURVEY***

Good day American Academy of HIV Medicine colleagues:

This post is for survey on health care practices. The goal is to better understand your experience providing health care to people with cognitive difficulties, and how equipped health care settings are regarding health care provision for patients with cognitive difficulties.

This study is not specific to HIV health care, but we are very much interested in hearing from professionals who serve patients with HIV.

We would like to hear from various health care disciplines: doctors, physician assistants, residents, fellows, nurses, psychologists, mental health/behavioral  health counselors, rehabilitation counselors, LMFTs, social workers, case managers, etc.

This is part of a VA and IRB approved study on health care access.

Title: Improving Health Care Access and Engagement for Veterans and Service Members with TBI Morbidity (I-HEAL).
Protocol Numbers: University of South Florida IRB Study #STUDY007220; VA IRBNet #1804543.
MPIs: Dr. Jolie Haun, Dr. Megan Moore, Dr. Risa Richardson.

    Participation is voluntary.

This brief survey should take no more than 15 minutes (less than 10 minutes in most cases). The survey can be accessed below:
