    57 yo woman with persisting viremia VL = 247.000 x months despite Biktarvy Rx. I checked her genotype and she has multiple mutations against PIs but none to the components of Biktarvy,  She has sensitivity to all components of Biktarvy.      Am I to asuume that her persisting viremia is simply due to non-adherence???

    Yes, that's the only plausible explanation. Of course, you have to make sure she's following the recommendations about divalent cations (dosing of antacids and supplements), but even if she were doing that wrong, you'd still expect more viral suppression than this.
