Dear Healthcare Providers,
My name is Rachel Kautz and I am currently completing a post-graduate first year community-based pharmacy residency with Walgreens and University of Illinois Chicago (UIC) College of Pharmacy. I am asking for your participation in the following research study survey. The goal of the study is to evaluate the knowledge and perceptions of healthcare providers with additional credentialing in HIV relating to vaccine coadministration, and to examine the responses based on personal and professional demographics.
The survey should take no more than 10 minutes to complete. Results will remain anonymous and confidential. Your participation is voluntary and you may withdraw from the survey at any point by exiting your internet web browser. By completing this survey, you are consenting to your involvement in this study. The research study has been approved through Howard Brown Health IRB.
